LIABILITY WAIVER AND AGREEMENT: I know that the sport of soccer is a potentially hazardous activity. I am aware and am thoroughly informed of the inherent risks of my child participating in the Excellence Through Fundamentals Soccer Camp (the “Camp”). I understand that there are dangers and risks inherent in these activities associated with the Camp. I attest that my child is in good health, has sufficiently prepared or trained for participation in the Camp, has not been advised by a qualified medical professional to not participate in the Camp or any similar activity or event, and that there are no health -related reasons or problems precluding participation in the Camp. I hereby assume all of the risks of my child participating in the Camp and, on behalf of my child and anyone claiming any interest through my child or on his/her behalf (the "Releasing Parties"), hereby waive, release, indemnify, and agree to hold harmless G.V. Sports, LLC D/B/A Excellence Through Fundamentals Soccer Camp and each of their member(s), Board members, employees, agents, subcontractors, representatives, attorneys, sponsors, volunteers, and all others assisting in the operation of the Camp and its supporting and related activities (the "Released Parties") from and against any and all actions, suits, claims, damages, and liability of any kind whatsoever and all costs thereof (including, but not limited to, attorney's fees and litigation costs) (the "Released Claims") arising out of or in any way connected with the Camp. I understand and assume all risk involved from my child participating in the Camp. I am aware that medical support for the Camp will be provided by volunteer personnel who may be called upon to provide assistance, including first aid, to my child during or after participation in Camp activities. I authorize any such volunteer to assist my child or to perform such assistance as in the opinion of such person may be necessary or appropriate. I agree that my insurance will provide primary coverage for any injury at the Camp. I hereby state that I am the parent/ legal guardian of my child legally authorized to execute this Liability, Waiver and Agreement on behalf of my child. I, on behalf of myself and my child, agree to obey and accept the rules of the Camp and any related events as published or otherwise made known to me, and to abide by the decision of any Camp official concerning my child ’s ability to safely participate and complete the Camp. I consent to the use of photographs, video, film and sound recordings of my child and/ or me in all events for all legitimate purposes. I give permission to be contacted by interested media. COVID -19: The Camp cannot prevent potential exposure to or infection by COVID - 19. By signing this agreement, the Releasing Parties acknowledge the contagious nature of COVID -19 and voluntarily assume the risk that they may be exposed to or infected by COVID -19 by attending the Camp. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Released Parties from any and all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating to COVID -19.
